What personal information do we collect?
We generally collect personal information about you (or your child/student as applicable) including:
- identification information (e.g. name and contact details like phone numbers, address/location and/or email address);
- spoken language other than English and ethnicity;
- health information (e.g. date of birth, gender and/or information about a diabetes condition or other disability, illness or condition); and
- your IP address and standard log information, such as your browser type, the pages and dates you accessed our systems and applications (e.g. cookies), your transactions and your other activities on our applications, websites and systems,
directly from you when you provide us with information, whether in person, by telephone, online or in writing or from the third parties below.
Personal information collected other than from you
We will collect personal information from you unless it is unreasonable or impractical for us to do so. For example, we may collect your (or your child’s) personal information from:
- service providers who provide services to us or on our behalf from time to time;
- the principal of the school that you attend (or your child attends), or you work at;
- a parent/guardian of a student that attends the school you work at;
- a staff member at the school you attend (or your child attends), or you work at; or
- a trainer or coordinator involved in the Diabetes in Schools program.

Why do we collect personal information?
We collect personal information as reasonably necessary for the purposes of administering, coordinating and delivering the Diabetes in Schools program, including to:
- identify principals, school staff, students and parents/guardians who may be interested in their school joining the Diabetes in Schools program, and identify, correspond with and establish a contact point in the relevant schools;
- communicate with you;
- register for training with the Diabetes in Schools program;
- facilitate, coordinate and deliver training on diabetes, through various means, including online and face-to-face;
- send promotional material and information about the Diabetes in Schools program (please see information on ‘opting out’ of marketing communications in our Privacy Policy);
- meet our legal obligations;
- inform you of any other initiatives or programs we think may be of interest to you;
- carry out account management;
- carry out our administrative, quality assurance and complaint handling activities;
- develop and implement initiatives to improve our services (including for the purpose of improving your user experience);
- contact you to enable us to manage your account and fulfil our obligations to you; and
- administer, manage and collect statistical information about the NDSS, including the Diabetes in Schools program and carry out any other activities under NDSS as approved or directed by government agencies (including the Department of Health and Aged Care).
Consequences if we do not collect your personal information
We may not be able to provide you with training on diabetes, access to training materials, or information on the Diabetes in Schools program.
Who do we usually disclose personal information to?
We usually disclose the personal information we collect to our related entities, service providers and contractors that help us administer the Diabetes in Schools program and supply our services to you which may include any of the following:
- our contractors (e.g. suppliers, marketing consultants, IT service providers and professional advisers such as legal practitioners, accountants and insurers);
- persons or organisations involved in the administration, coordination or delivery of training as part of the Diabetes in Schools program, including for example training coordinators, trainers, principals, other school staff, students and parents/guardians of students;
- The Department of Health and any other government department, as required by the Commonwealth Government;
- the AIHW for statistical analysis and research;
- NDSS Agents (as defined in our Privacy Policy); and
- other third parties, including organisations that deliver services on our behalf or to us, mailing houses and other organisations or government agencies for the purpose of administering the NDSS, including the Diabetes in Schools program.
We may also provide your personal information to other entities where required by law.
Disclosure of personal information to overseas recipients
We do not currently disclose your personal information to overseas parties. However if your personal information is ever transferred overseas, we will comply with our obligations under the APPs.
