    If you respond ‘Yes’ to any of the questions below, for the safety of yourself and others in the group, Diabetes Australia ask that you not attend this session and that alternative arrangements be made to provide diabetes support services.

    • Confirm that I will abide by the above behavioural etiquette.

    I understand that DiabetesAustralia may be required to share my personal information including my name and contact details with necessary governing health bodies including the relevant state or territory health departments for the purpose of contact tracing.

    PLEASE NOTE: This record will be kept confidentially on file for a period of 3 months and then destroyed.

 

Please fill in this information within 24 hours of the Level 3 Skills Training Event.

You may also do this manually, if you don’t have a phone, by requesting the form from the trainer on the day.

Contact Tracing

In the event that a person tests positive for COVID-19 and has been known to attend a Diabetes in Schools Level 3 training session, contact tracing may be required per state or territory guidelines.
